2. About the Opportunity:
The Patient Care Coordinator role supports physicians in delivering seamless, high-quality care by managing the administrative and operational aspects of their practice remotely. This role blends hospitality-level service with healthcare precision, ensuring patients receive attentive, proactive support while enabling physicians to focus fully on clinical care. The position is essential for creating a frictionless patient experience, anticipating needs, and resolving barriers in healthcare delivery. Success in this role directly impacts patient satisfaction and care quality by maintaining smooth operations behind the scenes.
3. Responsibilities:
• Provide responsive communication to patients, ensuring they feel supported and informed throughout their care journey
• Coordinate complex scheduling of patient visits, labs, and virtual meetings, managing physician calendars accurately
• Identify and resolve potential issues proactively to prevent disruptions for patients and physicians
• Detect and correct errors in documentation, scheduling, or processes before they affect care
• Develop organizational systems to help physicians prioritize tasks and manage responsibilities efficiently
• Build and maintain trusted relationships with external consultants and referral sources
• Manage clinical follow-through including prescriptions, lab orders, test tracking, and prior authorizations
• Suggest and implement workflow improvements to enhance patient coordination and operational efficiency
• Maintain thorough and accurate patient records, including medication lists and clinical documentation
4. Requirements:
• Experience in patient coordination, clinical administration, or a related healthcare support role
• Strong organizational skills with attention to detail and ability to manage complex schedules
• Excellent communication skills, combining professionalism with warmth and empathy
• Proactive problem-solving ability and comfort anticipating needs before they arise
• Ability to collaborate effectively with clinical teams and external partners
• Experience with electronic health record systems and clinical documentation is preferred
• Ability to work independently in a fully remote environment
